Dear Patients,
On January 31, 2023 I will be retiring from my private Family Medicine practice. It has been a privilege to have held your trust.
Your ongoing care remains my top priority, and I am pleased to announce that Primary Medical Group (PMG) has accepted custodianship of my records and will be available for any of my patients who wish to transfer and continue their care after the close of my office. Primary Medical Group has been a respected primary care physician group in Ventura County since 1994. They have seven locations in four cities to serve you as well as a website showcasing their physicians at www.primarymedical.net. Although it has been necessary to cancel any existing appointments you may have in my office after 1/31/2023, PMG at 10885 Telegraph Rd. Ventura, CA 93004 is accepting new patients and is prepared to welcome you into their practice for those of you interested in transferring to their care. Primary Medical Group can be reached at (805) 647-7704 to make an appointment for February 2023 or thereafter, or at (951) 225-6807 for help switching the Primary Care Physician listed on your subscriber card after January 16, 2023 for those of you with HMO insurance contracted to the SeaView IPA. As my office will not be available after 1/31/2023, any pending lab or imaging orders from my practice should be completed by January 16, 2023 to allow sufficient time for me to address results, or deferred to your next physician to re-order under their own name.

I am grateful that Primary Medical Group's experienced physicians, nurse practitioners and physician assistants are willing and ready to accept the responsibility to continue your care.
